Added install instructions.

…and use that, as well as the environment, to name resources.  Also use this name for the zip file and eliminate zipfile_name from config.

`update_event_sources` fails on SNS and S3 event sources

And, for debug, print the full response for error cases.

Per https://github.com/garnaat/kappa/issues/32 , adding an update method
for event sources that don't have them.
